Mock-Up Approval
Please carefully review the information on your labels for accuracy. If there are changes needed, note them on here. Also carefully review all terms and conditions. Sign and date pages two and five then fax all pages to 1-800-884-1656. We cannot begin printing until we receive written approval on your labels and signed consent to our terms and conditions.Please note: any changes required on labels after the original setup is approved are subject to a $125 per hour charge. Any major design work may require you to obtain the assistance of a professional graphic designer. Due to limited space, we can only print city, state, phone and website or email information on back of label. In some cases, on smaller labels, we may not be able to print both phone number and website or email address; in those cases we will choose one or the other at our discretion. The one-time graphics fee of $99 covers the labor required to set up each individual label for printing.
We urge you to complete this process as soon as possible so that we can begin work on your order immediately. Particularly if you have an opening date or other deadline to meet such as a scheduled training, last minute holdups may mean that your product won’t arrive in time to meet your deadline or will have to be shipped by expedited means. Changes requested after giving final proof approval are subject to a $125 per hour design fee and may require another mockup (via pdf or fax for formatting adjustments, via hard copy for color adjustments) and approval. We will not begin printing until we receive written approval. If we have already printed your order when we receive new changes, changes will be reflected on the next order. If you require that an order be re-labeled, this will be at your expense. We cannot be responsible for any errors inadvertently overlooked by you; please proofread carefully!
Although our digital label printing system produces very high quality output, areas of solid color may reveal any minor inconsistencies that may exist in both the print medium and the large format inkjet process as a whole. This is an inherent feature of this process and we always strive for the best possible print quality.
This Agreement may be executed via facsimile or this electronic form, all of which shall be treated for all purposes as original documents, and, taken together shall constitute one and the same instrument. An electronic signature is any electronic means that indicates that a person adopts the contents of an electronic message. Your acceptance of this document is by completing this form saves trees.
